Pricing for Clinical Services

Insurance

• We are an out-of-network clinical team and our providers do not process insurance claims directly. We will provide you with a Superbill for you to use when seeking reimbursement of services from your insurance company through the secure online patient portal.

• The superbill contains all of the information you will need for reimbursement.

• Please note: although many PPO insurance plans provide reimbursement, Quantum Integral Healing Arts cannot predict the rate of reimbursement your plan allows for. If you have questions, we urge you to contact your insurance company to confirm your plan details and anticipated reimbursement.

Payments

• Payment for services are due on the date when services are received unless otherwise determined between you and your provider:

• Credit Card Processing Fee: There is a 3% credit card processing fee for all credit card transactions.

• You may also pay by cash, check or Venmo account services upon request.

Medical Records Requests

• It is your right to have access to all your medical records upon request. All requests for copies of your medical records must be in writing; no telephone requests can be accepted.

• An appropriate fee based on the current hourly rate may apply for the preparation of a treatment summary.
